UIS-Telecommunications Telephone Order Form Instructions
Note: Only authorized persons in the respective department/school may submit a service order
request to UIS-Telecommunications Department. We can assist you in identifying the authorized
point-of-contact in your department/school by calling Customer Service at 5-8590 or 617-495-
8590.
Service Order Request Form is available on-line through the TORTE system
(Telecommunications On-line Reporting/Request Tool for End Users.)
The service order request form can be utilized to submit requests for new installations, moves, changes,
disconnects, and other common telephone service requests. To submit a request for a Merlin/Legend system
please use this form and e-mail the request to
telecom_orders@harvard.edu. For orders on the Longwood campus
please e-mail telecom@hms.harvard.edu.
Once we receive your telephone order, our Telecommunications Specialists will review and
process your order. You can expect your service request to be scheduled within 3 - 7 business
days from the date the order was processed. The due date interval may vary depending upon
volume of orders received in the office, time of year, and additional required information that is
needed for processing. Once the order is processed, you can expect to receive a confirmation
copy of the service order.
